Gather your records if you are new to the school district and preparing to register your child. Children are assigned to schools based on their address, but families may use school choice to apply to other schools.
What You Need
A parent or guardian must accompany students at registration. The following documents are required:
•A report card or copy of school transcript from the last school attended.
•Address verification, such as a current electric bill, a tax receipt or homestead exemption card, warranty deed or contract for home purchase.
•Authenticated birth date. Options include a certified copy of the child's birth certificate, the child's passport, a baptismal certificate that includes the date of birth and parents' sworn affidavit, school records for at least four years that show date of birth or an insurance policy on the child for at least two years.
•Immunization records.
Health Requirements
If you're coming from out of state or from any private school, you also must have a record of a physical examination from a physician or the Hillsborough County Health Department performed within the past 12 months.
All children must have received the minimum number of immunizations that the state requires. Vaccines vary based on grade but include immunizations for chicken pox, polio, hepatitis B, tetanus and other shots.
The district has partnered with organizations to offer free physicals and immunizations for school-age children entering Florida schools for the first time. Appointments are required at all locations, except for the University Area Community Health Center. Dates and locations are:
•9 a.m. to 1 p.m., Aug. 2: Walden Lake Medical Arts Center, 1601 W. Timberlane Drive, Plant City. Call (813) 754-4444 for an appointment. Mintz Elementary, 1510 Heather Lakes Blvd., Brandon. Call (813) 870-4747 for an appointment.
•9 a.m. to 1 p.m., Aug. 9: Shields Middle School, 3908 19th Ave. N.E., Ruskin. Call (813) 870-4747 for an appointment.
•Blake High School, 1701 N. Boulevard, Tampa. Call (813) 870-4747 for an appointment.
•9 a.m. to 1 p.m., Aug. 16: Town 'N Country Recreation Center, 6039 Hanley Road, Tampa. Call (813) 870-4747 for an appointment.
•University Area Community Health Center, 13601 N. 22nd St., Tampa. Walk-ins welcome or call (813) 870-4747.
